- Health reminders: medication reminders, managing medical, dental, optical appointments and health screenings, monitoring blood sugar, blood pressure, heart rate, etc.
- Wellness support: safety supervision, light exercise encouragement, cognitive stimulation, meal planning
- Socialization: reading a book, playing games or engaging with hobbies, accompanying you on walks or outings

- When you call our office, we will start by gathering basic information, we will ask questions to find out your wishes, then we will schedule an in-home visit to determine your needs and environment assessment as well as answer any questions
- We will work with you to create a plan of care taking into consideration your history, routines and preferences.

ABOUT US
HISTORY: SLS was founded in 2002 by Ms. Kathleen Chavez in Grants NM. In need of help for her sibling and after advocating for her own family members within complex care systems, she envisioned a service that combines bureaucratic expertise with heartfelt support. “Our mission is simple,” she says, “To ensure every senior feels valued, every family feels empowered, and no one navigates this journey alone”. She wanted to start a company that helped people in need with Activities of Daily Living. She worked hard with PCS and eventually added DDWaiver and DOL to the business. Her sister was Special Needs and non-verbal and was in an institute where she was abused so her goal was to bring her home and have a program to take care of her. She grew to be the largest company around. She eventually had a couple of investors, and the company grew from a one room office to a built from the ground up large fully equipped building for all three programs. Then in 2022, ready to retire, Ms. Kathleen sold the company to CNSCares, a company who specializes in in-home care for Former Energy Workers (DOL Program). Eventually SLS stopped providing services for the DDWaiver, but continued to provide Personal Care Services for individuals qualified through Medicaid Turquoise Care.
